Group Personal Accident Insurance

Singapore Sports School has taken up a Group Personal Accident policy for our student-athletes.

The policy offers basic coverage to help defray partially the costs incurred by the student-athletes due to accidents.

For claims arising from accidents taking place up to 31 Dec 2024, student-athletes should use the forms below:

For claims arising from accidents taking place up from 1 Jan 2025 onwards, student-athletes should use the forms below (Policy Number 3096000):

The completed claim form and supporting documents should be sent via email to managed_care3@singlife.com 

Do include the parent’s and/or claimant’s contact numbers and email addresses in your email so that Singlife may contact you directly with regard to the claim.
